Prescription services for eligible patients.
Membership-based services with recurring billing.

2. USER RESPONSIBILITIES

You must be physically located in a state where Altera Wellness operates at the time of consultation.

3. MEMBERSHIP AND PAYMENTS

Membership fees are billed on a recurring basis and are non-refundable once a billing cycle begins.
Prescription costs are charged separately and are non-refundable once processed.
You may cancel your membership at any time with written notice.

4. LIMITATION OF LIABILITY

Altera Wellness is not responsible for technical failures, missed diagnoses due to telemedicine limitations, or adverse medication effects.
We do not guarantee specific health outcomes.

5. DISPUTE RESOLUTION

Any disputes shall be resolved through binding arbitration in [Insert State].
